Requirements LOCUM- Trauma Surgery APP Looking for one provider to cover shifts January - ongoing through 2026. Specific shifts are being determined and will be shared here once determined. *UPDATE*- potential shifts: 1/20-1/30, provider will need do complete an orientation before. Please ensure the bids reflect the following: - Guaranteed hours: 8 per day - OT to apply after 8 hours per day OT will apply as the shift is 7a-6p. Surgical assisting preferred Please note: St Joseph Eureka Hospital Surgery and Trauma Department has APP coverage 7am – 5pm seven days a week. The shift is split on Friday to provide outpatient coverage in the general surgery clinic from 9a-12p. Active CA license and CA DEA are required. Please note that the privileging timeline will not begin until the RFA and application are turned back in. Typical turn around for RFA is 48-72 hours. Application should be submitted within 5 days. o Typical privileging timeline is 90 days o All HBS specialties require 2 years of current Hospital clinical activity in the specialty. o Gaps greater than 3 months must be explained. o Include current number of case logs upon presentation for surgical specialties. Please note the following travel deviations from our contract will apply to the assignment (or current confirmation agreement): • NOT reimbursed: o Mileage from provider’s home to departing airport, as well as from airport to home upon return. o Ride share (Lyft/Uber/etc.) and/or taxi from provider’s home to departing airport, as well as from airport to home upon return. • WILL be reimbursed: o Airport parking at provider’s departing airport if provider drives themselves to the airport. o Insurance policies for rental cars must be pre-approved, prior to booking, otherwise it will not be reimbursed. o Ride share (Lyft/Uber/etc.) and/or taxi from destination airport to hotel/lodging for assignment, as well as from lodging to hospital/clinic, and return trips.
